Photo & Video Consent Form

Volley Vibes Volleyball Club may take photos and videos of athletes during practices, games, and events. These images may be used to celebrate our athletes, promote our programs, and share updates with our community.

Where Media May Appear:

• The club’s official website

• The club’s official social media channels (Instagram, Facebook, YouTube, TikTok, etc.)

• Promotional materials such as flyers, newsletters, or posters

Consent:

By signing below, I, the parent/legal guardian of the child named below, give permission for Volley Vibes Volleyball Club to take and use photos and/or videos of my child for the purposes stated above.

I understand that:

• My child’s name will not be shared publicly without additional consent.

• The photos and videos may appear online and be accessible to the general public.

• I can withdraw this consent at any time by providing written notice to the club, and the club will make reasonable efforts to remove future use of my child’s image.

• This consent is voluntary and does not affect my child’s ability to participate in volleyball activities.
